Qualitative Improvement Initiative.. Management of Surgical Smoke in the Operating Room

The topic that I chose for Week 3’s, Executive Summary Assignment regards surgical smoke in the Operating Room and how to handle it efficiently and effectively without any harm towards the staff or patient present in the Operating Room. Surgical smoke is a by-product which appears in plumes, caused by open or laparoscopic surgeries (Croke, 2020). Exposure to this smoke is extremely hazardous, as it can cause many respiratory illnesses, as well as the smoke, can contain bioaerosols, chemicals, viable cancer cells, nonviable particles, blood, tissue, deadly viruses, and bacteria (Croke, 2020). These factors are dangerous, especially for the preoperative personnel, as they are the ones who monitor the patient after they wake from the anesthesia and are in the Operating Room for a more extended period, which means they are the most exposed to the plume. Health care workers are exposed to many chemicals, such as Cyanide, Hydrogen Cyanide, and Carbon Monoxide, which are all killer elements. The purpose in my executive summary is to find a way out of the dangers of Surgical Smoke, as they are the reason for many respiratory illnesses, such as Emphysema, Asthma, Chronic bronchitis, Hypoxia or dizziness, nasopharyngeal lesions, Sneezing, and Nose and throat irritation (AORN, nd).

My goal is to present successful evacuation systems from Surgical Smoke. In many hospitals in individual states, regularly practicing evacuation techniques is a mandated law, but in most others, it is not. My purpose is to convince hospitals, specifically ORs, to follow those laws and effectively prevent fatalities or lives affected by surgical smoke. According to Croke (2020), room ventilation with smoke evacuation is the first line of protection against surgical smoke (Croke, 2020). The second line of protection uses a face-piece respirator such as the surgical mask or N95 respirator mask. The cover is not reliable at the most as compared to smoke evacuation, but in the direst need, that should be followed effectively. Also, it is essential to remember that when the pneumoperitoneum is released during or at the end of the surgical procedure, a filtering device should be used to protect the perioperative team members from exposure to the surgical smoke. There are also smoke evacuators available for use to de-insufflate the abdomen using a closed system. The AORN reported a study regarding surgical smoke caused by spinal surgery and a laser to infuse the skin together. It produced surgical smoke, and when using proper tools to detect the nonviable particles with the surgical smoke pencil, the faculty and patients could effectively evacuate the premises without the smoke affecting anybody. The ventilation in the room, which takes in the polluted air, recycles it, and filters the harmful particles restoring the sober environment before the surgery. That is when it is clear to head back in and for the preoperative nurses to take care of the patient. It is evident that with proper evacuation and filtering techniques, the overall environment in the hospital is much improved.

I chose to base my executive summary assignment on surgical smoke because I am currently working in the Operating Room and am as exposed to Surgical Smoke as any other OR nurse. It also concerns because we perform surgeries on COVID 19 patients, and when smoke is omitted, the nurses can inhale particles, therefore giving us the virus. This is a developing and concerning issue at my hospital. In my state, it is not just yet, so the purpose of my goal is to make it a law and to carry it out effectively. Surgical smoke is a severe issue as it can be lethal for many; through proper management, precautions can help minimize the danger of it.
